CLAIM OF GROUP INSURANCE ON DEATH OF AN EMPLOYEE IN SINDH

In Sindh, the Group Insurance claim is payable after death of the Government Servant. Therefore, the heirs of deceased employee and Offices across Sindh will have to fill three types of Forms and other Certificates will also have to be made, so the details of Forms and other documents are presented More required Documents as under: a. Attested Copy of Death Certificate. b. Claim Form (Annexure-1) duly completed and verified by authorized officer. c. Attested copies of NIC / CNIC of deceased and Nominee /Recipient of family Pension / Successor / Guardian. d Original Nomination Form / Attested copy of Pension Book / Succession Certificate Issued by the competent Court of Law/ Heirship Certificate Issued by the Mukhtiarkar. e. Attested copy of complete Service Book or Last Pay Certificate or Computerized slips issued by A.G/D.A.O for ascertaining Grade/BPS of the deceased at time of death. f. If Nomination or Recipient of family pension is/are not available, then to submit the details of family members as per Sindh Civil Servants Welfare Fund Ordinance (Annexure “D”) g) Formal Application of Widow and FRC issued by the NADRA.

APPLICATION FOR GRANT FROM THE SINDH GOVERNMENT SERVANTS BENEVOLENT FUND

In Sindh, the Benevolent Fund is payable after death of the Government Servant. Therefore, the heirs of deceased employee and Offices across Sindh will have to fill Form Two types of Forms ( Part-I & II i.e Non-Gazetted and Gazetted ) and other Certificates will also have to be made, so the details of Forms. At Present We are uploading a Form Part-II for Non-Gazetted and very soon Part-I uploading on web for facilitation of Employees of Sindh . MONTHLY MAINTENANCE ALLOWANCE FORM FOR GAZETTED PART-I & NON GAZETTED PART-II Monthly Claim Form download for monthly payment of beneficiary. Monthly claim form may be attested from e Gazetted Officer of (BS-17 & above) Government of Sindh. While applying for the grant, download the(*application form &*Descriptive Roll)and submit it with the following documents: Prescribed APPLICATION FORM duly countersigned by Secretary or Additional Secretary Head of the Administrative Department of deceased Government servant (original). Attested copy of CNIC of applicant & deceased Government Servant. Attested copy of DEATH CERTIFICATE issued by NADRA Attested copy of HEIRSHIP CERTIFICATE issued by competent authority Attested copy of RETIREMENT ORDER or OBITUARY. Attested copy of NO-MARRIAGE OR LIFE OR DEPENDENCY CERTIFICATE issued by Union Council. Attested copy of DESCRIPTION ROLE (in original). Attested copy of CHEQUE BOOK LEAVE of bank account of applicant (No joint Account accepted). Attested copy of FINAL G.P FUND SLIP. Attested copy of LAST PAY CERTIFICATE or proof of Benevolent Fund deduction. Pension Roll Slip & Pension Data Roll Sheet (Treasury Office or AG Sindh) Note:-All documents should be completed and completely form filled otherwise the form will be rejected. MARRIAGE GRANT FORM NON GAZETTED PART-II Submit the downloaded application form along with the following documents: Prescribed APPLICATION FORM duly countersigned by Secretary or Additional Secretary Head of the Administrative Department of deceased Government servant (original). Attested copy CNIC Government Servant and Bride. Attested photocopy Office Card. Detail of family member such as their names, ages, relationship, married / un-married, school/college & occupation. Attested photocopy of NIKKAH NAMA Marriage invitation Card Original. Certificate that: this is your first request grant of marriage expenses and you have not drawn aid from benevolent fund for Marriage Expenses has granted marriage expenses prior to this application, none of family member has granted marriage expenses out of benevolent fund office in province of Sindh. Attested copy of CHEQUE BOOK LEAVE of bank account of applicant Attested copy of DESCRIPTION ROLE (in original). Affidavit Rs. 50 (or easily available in market or from the Bank). مشتری ھوشیار باش! ادائے خاص سے غالِب ہُوا ہے نُکتہ سرا۔ صلائے عام ہے یارانِ نُکتہ داں کیلِئے ۔ بجٹ تقاریر میں ملازمین کو ملنے والے رلیف اور شوشل میڈیا پر حکومتی اعلان کے بعد اکثر ملازمین ساتھی اپنی اپنی تنخواہوں میں اضافہ کا پوچھ رہے ہیں جب کی تنخواہ میں اضافے کی رقم جولائی 2023 والی تنخواہ جو کہ پہلی اگست 2023 میں ادا ھوگی اس کے ساتھ ملے گی اور اس سے پہلے حکومت کی جانب سے نوٹیفکیشن جاری ھونا ھے ۔ ساتھیو ! وفاقی اور صوبہ سندھ کی بجٹ 24-2023 میں ملازمین کو رلیف دینے والی تجاویز کو حکومت پاکستان اور سندھ نے جان بوجھ کر مبھم رکھا ہے اور بجٹ دستاویز سے لگتا ھے کہ دال میں کالا نہیں ہے بڑا گھوٹالا ہے کیوں کہ بجٹ کاغذات میں کوئی واضع تجاویز نہیں کہ یہ ایڈھاک رلیف الائونس اسکیل 2022 کی بنیادی تنخواہ پر دیا جائے گا یا ماضی کی طرح اسکیل 2017 پر ھوگا ۔ بجٹ دستاویز میں خالی جگھ چھوڑی گئی ہے۔ یہ پہلی مرتبہ دیکھا ھے۔ یاد رہے کہ دوہرے معیار کو حکومت جس طرح پہلے ہی مضبوط کر چکی ہے جس میں سیکریٹیریٹ کے افسران کو %150 فیصد ایگزیکیوٹو الائونس , عدالت عظمئ کے ملازمین کو %100 فیصد اسپیشل جوڈیشل الاؤنس , قومی اسیمبلی ممبران کو فیوئل الائونس %100 اور دیگر کو مئی 2023 میں رلیف دینا یا اس سے پہلے ھائوس ھائرنگ میں چوتھی بار اضافہ کرکے مخصوص سیکٹر کے ملازمین کو رلیف سے نوازنا ؛ یوٹلٹی الائونس میں اضافہ و دیگر مراعات دیکر انکو سول ملازمین کے معاشی قتل کے Death Warnt پر مکمل راضی کیا گیا ھے۔ اس لئے ھوشربا مہنگائی میں نا ھائوس رینٹ الائونس میں اضافہ ھوا نا کنوینس الائونس کو بڑھتی ہوئی پیٹرول کی قیمتوں کے برابر کیا نا %300 فیصد ادویات کی قیمتوں کے برابر میڈیکل الاؤنس دیا گیا ھے ۔ ھمارا مشورہ ہے تنخواہ میں اضافے کی بنیاد پر قرضہ نہیں اٹھانا ۔ اس اضافے کی رقم سے ٹیکس کی کٹوتی بھی ھوگی۔ عام طور پر ملازمین اتنا سالانہ بچت نہیں کرتا جتنا ٹیکس ادا کرتا ھے۔ جب کہ ھائوس رینٹ , کنوینس اور میڈیکل الاؤنس پر ٹیکس لاگو کرنا بڑی ناانصافی ھے ان الاؤنسز کو ٹیکس سے مستثنیٰ کرنے کی ضرورت ھے۔ اس کے علاؤہ بلاول بھٹو زرداری کی ملازمین کی تنخواہوں میں %50 فیصد اضافے کی تجویز کو بھی نظر انداز کیا اور گورنر سندھ کے تجویز تھی کہ کم سے کم اجرت 50 ھزار کی جائے اسکو ردی کی ٹوکری میں ڈال دیا گیا۔ جب کہ مہنگائی %500-300 فیصد کی بلند سطح پر ہے ۔ منجانب ۔ ایپکا پاکستان شہید ساقی www.apcapk.com

MemberISecretary 6) Any Member to be co-opted by the Committee Page 1 of 2 Terms of Reference (TORS) of Upgradation Committee i. All requests for seeking ~p~gradationiredesignation will be sent to the above committee. . . 11. The committee will consider upgradationiredesignation matters on case- to-case basis received from Ministries / Divisions. iii. Departments should route the cases~thtough line Ministries / Divisions: iv. The committee will decide on the cases for upgradationlredesignation of the posts in BPS 1 to BPS 19. The Division concerned as per parameters of existing policy shall submit the cases of upgradation of posts in BS 20 and above for review by the committee before submission to Prime Minister along with the recommendations of the committee. v. Joint Secretary (Regulation), Establishment Division shall be the Secretary of the committee and will provide secretarial support to the Committee. NB: - The committee shall meet on a quarterly basis to decide the cases of upgradation. However, the chairman of the committee may convene the meeting at his discretion.

Adhoc Allowance with effect from 1 July 2024 as Granted by Federal Government, Provincial Government Sindh, Punjab, Balochistan as Under: Federal Allowed 25% from BPS-1 to 16 and 20% from BPS 17 to 22. The Punjab Government has also given relief of Adhoc Allowance on the federal pattern and Sindh has given slightly different granted an ad-hoc 30% to Grades 1 to 6 and 25% adhoc to Grades 7 to 16 and 22% adhoc to Grades 17 to 22 employees.
