APPLICATION FOR GRANT FROM THE SINDH GOVERNMENT SERVANTS BENEVOLENT FUND

In Sindh, the Benevolent Fund is payable after death of the Government Servant. Therefore, the heirs of deceased employee and Offices across Sindh will have to fill Form Two types of Forms ( Part-I & II i.e Non-Gazetted and Gazetted ) and other Certificates will also have to be made, so the details of Forms.
At Present We are uploading a Form Part-II for Non-Gazetted and very soon Part-I uploading on web for facilitation of Employees of Sindh .

MONTHLY MAINTENANCE ALLOWANCE FORM FOR GAZETTED PART-I & NON GAZETTED PART-II

Monthly Claim Form download for monthly payment of beneficiary. Monthly claim form may be attested from e Gazetted Officer of (BS-17 & above) Government of Sindh.

While applying for the grant, download the(*application form &*Descriptive Roll)and submit it with the following documents:

Prescribed APPLICATION FORM duly countersigned by Secretary or Additional Secretary Head of the Administrative Department of deceased Government servant (original).
Attested copy of CNIC of applicant & deceased Government Servant.
Attested copy of DEATH CERTIFICATE issued by NADRA
Attested copy of HEIRSHIP CERTIFICATE issued by competent authority
Attested copy of RETIREMENT ORDER or OBITUARY.
Attested copy of NO-MARRIAGE OR LIFE OR DEPENDENCY CERTIFICATE issued by Union Council.
Attested copy of DESCRIPTION ROLE (in original).
Attested copy of CHEQUE BOOK LEAVE of bank account of applicant
(No joint Account accepted).
Attested copy of FINAL G.P FUND SLIP.
Attested copy of LAST PAY CERTIFICATE or proof of Benevolent Fund deduction.
Pension Roll Slip & Pension Data Roll Sheet (Treasury Office or AG Sindh)
Note:-All documents should be completed and completely form filled otherwise the form will be rejected.

MARRIAGE GRANT FORM NON GAZETTED PART-II

Submit the downloaded application form along with the following documents:
Prescribed APPLICATION FORM duly countersigned by Secretary or Additional Secretary Head of the Administrative Department of deceased Government servant (original).
Attested copy CNIC Government Servant and Bride.
Attested photocopy Office Card.
Detail of family member such as their names, ages, relationship, married / un-married, school/college & occupation.
Attested photocopy of NIKKAH NAMA
Marriage invitation Card Original.
Certificate that: this is your first request grant of marriage expenses and you have not drawn aid from benevolent fund for Marriage Expenses has granted marriage expenses prior to this application, none of family member has granted marriage expenses out of benevolent fund office in province of Sindh.
Attested copy of CHEQUE BOOK LEAVE of bank account of applicant
Attested copy of DESCRIPTION ROLE (in original).
Affidavit Rs. 50 (or easily available in market or from the Bank).
Pay slip
Note:-Submit the application form may be route through the Head of Administrative Department to Benevolent Fund within 01 years

FUNERAL GRANT FORM FORM NON GAZETTED PART-II

Submit the Downloaded application form to avail the grant and submit it with the following documents:

Prescribed APPLICATION FORM duly countersigned by Secretary or Additional Secretary Head of the Administrative Department of deceased Government servant (original).
Attested copy CNIC Employee/Applicant/Deceased Government Servant.
Attested photocopy Office Card.
Attested photocopy of NADRA DEATH CERTIFICATE
Detail of family member such as their names, ages, relationship, married / un-married, school/college & occupation. .
Attested copy of CHEQUE BOOK LEAVE of bank account of applicant
Attested copy of DESCRIPTION ROLE (in original).
Affidavit of Rs.50 (or as available of any cost in market)
Pay slip.
